This problem affects all versions of Unified Care Record and Information Exchange prior to 2026.2.
Requirements:
- IHE XDS.b
System Area Impacted:
- Patient Document Storage, Replacement, and Deletion
CDA and CCDA documents with a replacement code:
<relatedDocument typeCode="RPLC">
can cause previously ingested patient data to be cleared when later documents are processed.
This issue can occur in the following cases:
- Query-based Edge Gateways such as Notify and Query
- Multi-modal document ingestion workflows
- Deployments where replacement logic is managed outside of the CDA transform
This behavior has been addressed in version 2026.2, and an update is available via ad hoc patch for older versions.
The update adds a new optional import configuration parameter, blockPatientReplaceActionCode, that prevents unintended patient data clearing in CDA and C-CDA Imports. The setting is available for CDA, C-CDA R1.1, and C-CDA R2.1 imports. The default value of the setting preserves legacy behavior for backward compatibility.
