Manifest MedEx (MX) has worked to securely and reliably exchange health data across California since 2017. As the largest nonprofit health data network in California, it is an integral part of the state's health data infrastructure, combining and delivering cruicial health information for 38 million individuals across every county. Its mission is to providce every ehalthcare organization in California with the information they need to improve care, enhance health, and lower costs. MX is taking bold steps towards health transformation, working with partners to advance health equity, support public health initiatives, and achieve Medi-Cal goals.

Formed in 2018, Big Sky Care Connect is a nonprofit created to respond to the need for a state-wide coordinated health information exchange (HIE) and related services to enhance clinical care in communities throughout Montana.

Big Sky Care Connect serves to facilitate timely and secure access by health care providers to clinical information between various organizations and engage in other activities to improve and promote the public health and quality of health care in Montana in a cost-effective manner.

Healthconnect Texas (HTX) is an independent non-profit health information exchange (HIE) organizaition serving Texas, and is one of the largest public HIEs in the United States. Healthconnect facilitates clinical integration across the care continuum by connecting disparate electronic health record systems across the region’s hospitals, health systems, integrated delivery networks, community clinics, physician office practices, and service providers.

CyncHealth is a statewide health information exchange (HIE) designed to share clinical and administrative data among providers in Nebraska and neighboring states.

Alabama One Health Record® (ALOHR), Alabama’s health information exchange (HIE), lets doctors, hospitals, clinics, nursing homes and other healthcare providers share medical information electronically so that they can provide their patients with the best of care. ALOHR provides the necessary infrastructure to securely exchange electronic health records between authorized participants. Participating providers are able to query the interoperable, two-way data exchange from within their electronic health record system to access patient health data from other providers. Providers without an EHR system can access a secure messaging system.

Together, they create a trusted network to facilitate the efficient use and exchange of patient data to support patient-centered health care and continuous improvements in access, quality, outcomes and efficiency.

Tennessee Health Information Exchange (TnHIE), a non-profit community health information exchange, was established in 2005 to serve the data exchange needs of the healthcare community. TnHIE provides a secure technology platform for exchanging health information within its Community of Care, giving authorized providers immediate access to patients’ records from providers across the community, including hospitals, provider practices and other healthcare related entities. TnHIE is based in Knoxville, Tennessee and makes health information exchange available to all members of the healthcare community in all 95 Tennessee counties, as well as individuals from other states who receive care through TnHIE participants.

NC HealthConnex is North Carolina’s state-designated health information exchange (HIE) – a secure, standardized electronic system in which providers can share important patient health information. As a modernized health information exchange, NC HealthConnex brings added value to health care conversations happening at all levels in the health care industry. It is breaking down information silos between health care providers, achieving greater health care outcomes for patients and creating efficiencies in state-funded health care programs such as Medicaid.

NC HealthConnex was established in 2016. It is operated by the North Carolina Health Information Exchange (part of the N.C. Department of Information Technology) and coordinates closely with the N.C. Department of Health and Human Services.

Michigan Health Information Network Shared Services (MiHIN) is a public and private non-profit collaboration dedicated to improving the healthcare experience, improving quality and decreasing cost for Michigan’s people by supporting the statewide exchange of health information and making valuable data available at the point of care. MiHIN evolved as a solution to data silos that emerged among hospital and health systems and the variety of electronic health record software they implemented. Michigan subsequently formalized MIHIN as the state designated HIE and today engages with MiHIN as a consolidated, statewide, public-private partnership. As of 2024, 17 million data points flow through the statewide network each week.

Healthix is the largest public health information exchange (HIE) in the United States, serving New York City and Long Island. Healthix data for more than 21 million individuals is sourced from a diverse array of care providers, including more than 9,000 primary care practices, hospitals (emergency, inpatient, and outpatient), FQHCs, labs, radiology centers, behavioral health and community-based facilities, and more.

Hixny is a nonprofit health information network (HIN) in eastern New York that serves every provider, resident and visitor who accesses healthcare between Canada and New York City, and from Binghamton into Vermont.

The Pennsylvania eHealth Partnership is responsible, under Act 76 of 2016, for the creation and maintenance of Pennsylvania's secure health information exchange, known as the PA Patient & Provider Network, or P3N. The P3N exchanges data among five P3N certified regional health information networks (HIOs) across Pennsylvania and the Pennsylvania Department of Corrections (DOC).

The P3N improves and coordinates patient care by helping providers find patients' medical records—in real time—anywhere on the P3N network. Patients and providers benefit from these connections through reduced redundancy of tests, better coordination of care, improved patient safety, and lower costs.

In state fiscal year 2024, P3N processed more than 62 million Admission/Discharge/Transfer messages and nearly 14 million document retrivals.

The Georgia Health Information Network, or GaHIN, is a nonprofit organization dedicated to creating a healthier Georgia through the use and exchange of electronic health information. GaHIN is the state-designated entity for health information exchange (HIE).

GaHIN facilitates the use and secure exchange of patient health information so providers have the information they need at the point of care, resulting in improved quality of care, better health outcomes and reductions in cost.

By establishing a trusted network with existing health systems and health information networks, GaHIN facilitates the delivery of the right health information to the right place and provider at the right time—providing Georgia residents with safer, timelier and better care.
