Healthcare Interoperability Has Reached a Turning Point 

The 2025 CHIME Digital Health Most Wired Interoperability & Population Health Report, sponsored by InterSystems, surveyed 278 healthcare organizations representing nearly 40% of U.S. hospitals. The findings are clear: The industry has largely solved for connectivity. The defining challenge is no longer data exchange, but whether shared data is governed, standardized, and trusted enough to change what happens at the point of care.

Key Findings

  • 45% of health systems cite lack of trust in data accuracy as a primary interoperability barrier. 

    The Impact: Even with widespread connectivity, nearly half of organizations still can’t rely on their data to make decisions. 

  • 70% are connected to a QHIN, yet only about half describe their interoperability as fully integrated into clinical workflows. 

    The Impact: Being connected and being integrated are not the same thing. The gap between them is where clinical value is lost.  

  • 39% have a formally chartered data governance function. Leading organizations are 4× more likely to have multidisciplinary governance with executive approval. 

    The Impact: Governance is one of the biggest predictors of interoperability maturity; yet most organizations still don't have it in place.
