Delivering the risk scores to the emergency department supports critical discharge, admit and follow up decisions. Initially, the ED department received only a 30-day revisit risk score. However, upon reviewing the 30-day scores with the ED physicians, they were very interested in both intra-ED episode risks for mortality, as well as post discharge risks for disease and mental health exacerbations.

In this session, HBI Solutions CEO, Eric Widen will talk about how intra-episode risk scores, including sepsis and mortality, and post discharge risks for thinks like heart attack, stroke and suicide attempt can be delivered and support critical decisions made by ED physicians every day in emergency departments across a connected community.
