InterSystems Launches HealthShare® Care Community to Coordinate Personalised Care Across Distributed Healthcare Systems

Extending InterSystems HealthShare proven interoperability capabilities, solution connects clinicians, caregivers and patients to improve patient outcomes.

Eton, 24 November 2020. InterSystems, a creative data technology provider dedicated to helping customers solve the most critical scalability, interoperability, and speed problems, has today announced the launch of HealthShare Care Community™. The solution means care teams can create a comprehensive shared care plan – improving communication, care transitions, and care coordination.

With the power of InterSystems HealthShare behind it, HealthShare Care Community is helping patients, from the elderly to individuals living with long term, or chronic conditions, benefit from improved communication throughout their care journey. The solution creates comprehensive, flexible, and personalised care plans, combining input from the care team with additional wellness goals added by the patient. Plans can be customised to include wide ranging aims, from helping patients walk unaided for a family function, to end of life choices.

When using HealthShare Care Community, everyone on the team can see who is contributing to the patient's care, the role they serve, when they’re involved, and their contact information. The entire care team – from patients and clinicians to GP’s and community health workers – can easily contribute to a highly detailed, accurate and continually updated view of each patient’s condition and care preferences, within their native workflow.

With Care Community working in collaboration with the HealthShare product suite, providers can overcome the fundamental challenge for care teams that don’t typically interact when spread across various care settings, with the ability to communicate through the HealthShare clinical message centre. Further, patients themselves can also log in via HealthShare Personal Community to communicate with care providers, get access to prescriptions and flag when they need help.

Teams can also be more proactive with automatic notifications of patient-related events such as admissions, discharges, or transfers. What’s more, supported by HealthShare Health Insight, they can identify patients who fall into high-risk categories and manage their care using intelligent, data-led insights in a way that enables faster diagnosis, as well as better and more timely response to changes in patient condition and care coordination requirements.
