Advisory: A Second Care Plan Using the Same Patient and Template Prevents Deletion of the Original Care Plan

This problem affects the following products:

Requirements:

  • Customers who create a second care plan for a patient using the same template as an existing care plan

In the following scenario, a care plan will be unable to be removed from a patient record:

A user creates a care plan for a patient, enters data in that care plan, and then marks the care plan active and then final. Subsequently, a user creates a new care plan from the same template for the same patient.

This represents a clinical safety issue if incorrect data was entered into the original care plan, as that data can no longer be deleted, resulting in a patient having data in their record which should not be associated with them.

To mitigate this issue, users should indicate in the new, correct care plan, which data in the original care plan should not be considered as part of the patient record.

A correction for this issue is under development and will be announced in a subsequent advisory when it is ready. No timeline is available for the correction.
