InterSystems and Healthfirst Recognized by KLAS with 2025 Points of Light Award for Improving Continuity of Care After Acute Events

Real-time data exchange detects when a Healthfirst member is discharged from the hospital after an acute event and delivers encrypted alerts to healthcare providers to support timely care

Boston, Mass., June 10, 2025 – Providing timely care after an Emergency Department or hospital discharge related to mental health and substance use is a national challenge due to delays in claims processing. InterSystems, a creative healthcare data technology provider managing more than one billion health records globally, and Healthfirst, one of the nation’s largest not-for-profit health insurers, have been recognized by KLAS for their collaboration to address this challenge by creating a real-time data exchange.

The Points of Light Awards spotlights payer-provider partnerships that advance value-based care, reduce inefficiencies, and drive better outcomes through technology innovation.

Healthfirst’s Health Information Exchange (HIE), was built and expanded using InterSystems technology. The exchange identifies members requiring care after an in-patient stay or Emergency Department visit pursuant to time-sensitive Healthcare Effectiveness Data and Information Set (HEDIS) measures. The real-time exchange began with a single measure and has been expanded to support seven additional time-sensitive measures.

In 2024, the program resulted in improvements across time-sensitive HEDIS measures when comparing follow-up care for acute events triggering an alert to those with no alerts. Results indicated that the alerts enabled clinicians to provide timely care to patients in need:

  • Follow-Up After ED Visit for Mental Illness: 13 percentage point lift
  • Follow-Up After ED Visit for Substance Use: 8 percentage point lift
  • Transition of Care medication reconciliation: 9 percentage point lift

In addition to improving post-discharge continuity of care, providers also saw reduced manual workload by embedding alerts directly into operational workflows.
