Advisory: Orphaned Care Plans After Multiple Patient Merges

When multiple patients (three or more) are merged together in InterSystems EMPI (formerly, HealthShare Patient Index), care plans that should belong to the merged patient can sometimes be rendered inaccessible in Care Community.

This specifically happens if the source patient in the first merge was first opened in Care Community before the target patient. After the merge, care plans belonging to any patients merged into this target patient in the future will not be visible when opening the final merged patient in Care Community.

The correction for this defect is identified as HSEUA-1994, which is included in HealthShare Care Community 2025.2 and all future product releases. It is also available for older versions via ad hoc change file (patch) or full kit distribution by contacting the Worldwide Response Center (WRC).

If you discover that you have inaccessible care plans, contact the WRC, who will walk you through a set of steps to clean up your existing inaccessible care plans, restoring them to that patient's care plan list.
