Longitudinal Health Record for Unified, Real-Time Insight and Better Care
A longitudinal health record (LHR) is a compilation of health and care activities – enrollments, diagnoses, treatments, payments, and other data, over time and across the health and care ecosystem. InterSystems HealthShare Unified Care Record® (HealthShare) is InterSystems LHR, the AI-infused foundation for your digital health platform, built to enrich and connect your many data sources to enhance care coordination, support value-based care initiatives, power analytics, and provide real-time insights.
Advanced Digital Health Foundation for Better Insight
The health and care ecosystem is diverse, and dispersed across a wide range of organizations, geographies, and disciplines. Similarly, the information this ecosystem creates and consumes is diverse, dispersed, and dynamic. HealthShare helps providers, health plans, consumers, social care and wellness services, researchers, and solution developers overcome the challenges of decentralized data. HealthShare delivers relevant data from the unified record into clinical and business workflows, communicates across your application and business ecosystem, and facilitates timely action through its AI Assistant and real-time notifications.
HealthShare data is aggregated in a normalized patient-centric data model so all of it, regardless of source, is stored in a single, consistent format with access controlled through strict consent management policies. It can be used to enhance clinical and business workflows, or to power analytics applications, HL7® FHIR® (Fast Healthcare Interoperability Resources) applications, and machine learning for knowledge discovery. Because it is updated in near real-time, you can be confident you’re always working with the latest information.
Key Highlights
An Extensible Health Data Model for the Whole Person, At Population Scale
HealthShare is based on a comprehensive and extensible health data model and data normalization technology for unifying all information relevant to each person, regardless of its source or format. This includes patient-generated data, clinical content, claims, and social determinants of health (SDOH), as well as other information types deemed relevant by your organization for your use cases.
Designed to support the world’s largest and most complex healthcare organizations and health information networks, HealthShare routinely ingests and makes available unified data for populations of tens of millions of patients.
Informing Clinical Care with the Power of AI
HealthShare, and the HealthShare AI Assistant, make comprehensive health data available to clinicians, care coordinators, case managers, utilization reviewers, and researchers wherever they work, in the format of their choice.
Equally accessible from desktop and mobile devices, the Clinical Viewer makes HealthShare data available via browser or embedded within your workflow tools of choice. The Clinical Viewer displays information in a readily digested summary format with click-through to detailed data. Configuration settings let organizations address specialty requirements, and the vital signs graphing feature allows individual users to dynamically configure their views for better insights from critical data.
Easily configured pre-defined Patient Reports offer another way to access relevant data for an individual.
The HealthShare AI Assistant lets you query longitudinal records for information of interest using simple text queries. The Assistant’s no-code prompt engineering tools help you rapidly and easily set up and deploy just the right query and formatted response for yourself, or a group of users, for example, to surface gaps in care or identify eligibility for a clinical research study.

Actionable Analytics for Individuals and Populations
HealthShare supports coordinated care across the entire care team, including consumers, with secure messaging and automated notifications. Sophisticated business logic capabilities let you define population cohorts of interest – for example, diabetics whose A1C is high. This means, for example, that care managers can subscribe to alerts on their members for events such as Emergency Department visits, abnormal lab results, gaps in care, and missed appointments. Such notifications help avoid costly adverse events and align care teams, population health managers, and patients around desired outcomes.
Single Sign-On (SSO) from Third-Party Applications
To minimize the cognitive burden on users, HealthShare supports single sign-on (SSO) between workflow systems—such as electronic health records (EHRs) or care management platforms—and HealthShare. With minimal configuration, administrators can create links that allow users to launch directly into any HealthShare clinical application without needing to log in again.
Beyond external integrations, HealthShare also provides seamless internal SSO. Once authenticated, users can move across HealthShare applications withoutre-entering credentials—even if those applications are hosted on different servers, data centers, or regions. This ensures a consistent, secure, and unified user experience across the entire HealthShare ecosystem.

Interoperability, APIs, and Advanced FHIR Capabilities
Whether as part of your data pipeline for analytics, AI, and machine learning, to feed other applications, or as part of mandated interoperability workflows, HealthShare can serve up data via APIs in general, and FHIR in particular. HealthShare reflects InterSystems longstanding healthcare interoperability leadership with concurrent support for FHIR R4, and STU3. Its FHIR support includes:
- Representing all your data as FHIR resources, and using our FHIR Server to simplify data sharing with devices and applications.
- Mapping between older data formats and FHIR to help you increase the value of legacy systems.
In addition to FHIR, HealthShare supports all the major national and international interoperability standards, including HL7v2, X12, and multiple CDA formats, that are used by your data sources to send data to HealthShare, or receive it from the LHR.

Management Portal
The Management Portal is a centralized administrative interface for configuring, monitoring, and maintaining the HealthShare environment. Administrators can manage user provisioning, SSO and general security options, as well as define granular consent policies to ensure compliance and governance. The portal also enables configuration of messaging and notifications, monitoring and debugging of system operations, and data management at population scale—supporting environments with hundreds of millions or billions of records. By consolidating these capabilities into a single, secure interface, the Management Portal simplifies administration while ensuring reliability, scalability, and control.
