Global Shared Care Records 2025

KLAS Research - Healthcare IT Insights

Successful HIE Deployments Outside the US

Introduction

Across the globe, data sharing is being facilitated across the continuum of care as regional groups (i.e., loose affiliations between neighboring healthcare organizations, formalized regional initiatives, country-wide initiatives) form to support the use of shared care record technology. A shared care record, also known as a health information exchange (HIE), is a system with the primary purpose of enabling information exchange between care organizations and/or social institutions that use various care records. Adoption of this technology varies widely across global regions and is growing thanks to various government or regional initiatives. For this report, KLAS interviewed 22 organizations (representing 14 regional shared care records in 6 countries outside the US) identified by shared care vendors as customers who have successfully used the technology to facilitate patient data exchange. This report looks at both the scale and scope of data exchange across these networks to provide insights into benefits, cost, connected settings, interoperability standards, and best practices. The complete case studies can be found in the full report.

Focus of the Global Shared Care Records 2025 Report

Shared care records often follow a hub-and-spoke model, connecting all the organizations within a network. The focus of this report is on the standalone HIE technology supporting these networks to facilitate broad data sharing between unaffiliated organizations using multiple care records. Note that this report does not represent a comprehensive census of all shared care records; rather, it is intended to help provide insights into successful data-sharing collaborations.

Data Integration Key to Achieving Needed Breakthroughs in Healthcare When peers from other industries ask why data hasn’t driven comparable breakthroughs in healthcare, the answer is usually hiding in plain sight: clinical and administrative data still live in different systems and “speak” different digital languages. Integrating them is a prerequisite for the next wave of progress—better care, better patient experience, and better operational performance at the same time. Fortunately, there are now multiple mandates spurring this integration, from Centers for Medicare & Medicaid Services requiring Fast Healthcare Interoperability Resources APIs, to National Committee for Quality Assurance embracing digital quality measures, to enforcement of prohibitions against information blocking. Health plans and providers, however, must guard against swapping these two longstanding data siloes for several new ones.

Overview An eye surgery center was losing patients. Not to competitors, but to no-shows. Eighteen percent of scheduled appointments went unfilled every week. Four full-time staff members spent their days calling roughly 500 patients, trying to confirm appointments. When that didn't work, the clinic overbooked randomly, hoping the math would balance out. Some days every patient showed up, and the waiting room was chaos. Other days slots sat empty.

Agentic AI in Healthcare A multi-agent AI system recently diagnosed rare medical cases correctly over 80% of the time. That's four times better than seasoned human clinicians. It didn't get there by being smarter than any single AI. It got there by having multiple agents argue with each other until the errors got caught.
