The National Million Hearts Campaign has a goal of preventing 1 million heart attacks and strokes by 2017 and identified high blood pressure or hypertension (HTN) control as an area of focus. Key strategies for this include identifying adults with HTN that are currently undiagnosed, moving those being treated for HTN toward control, supporting self-management among adults with HTN and driving the measurement and reporting of HTN.

The New York State Department of Health (NYSDOH) pledged a commitment to the Campaign and has engaged in a number of initiatives that align with the key strategies in the national plan for blood pressure control. One major focus has been improving the availability of local data to support public health and clinical practice. The NYSDOH has developed and piloted a method for identifying undiagnosed hypertension in patients receiving care at three federally qualified health centers using the center’s EHR data and collaborated with NY State’s Quality Improvement Organization to promote reporting an endorsed measure of HTN control to the Centers for Medicaid and Medicare Services (CMS).The clinical information within Regional Health Information Organizations (RHIOs) has considerable promise for estimating population health indicators for small geographic areas.  This information could assist local health departments, hospitals, health care providers, health plans, federally qualified health centers and other partners in identifying high need populations, developing a region-wide plan to reduce the burden of HTN, and connecting individuals with community and clinical resources.

Its programs and services, designed around the Hixny Health Information Exchange (HIE), give patients and clinicians real-time electronic access to patients’ comprehensive medical history for making timely, informed care decisions. Hixny is a national leader in provider HIE adoption.

During this webinar, we will:

  • Discuss the federal Million Hearts initiative as an example of a multi-sector population health improvement program.
  • Understand the process steps involved in a pilot to use data collected by a health information exchange to estimate population health measures related to the Million Hearts initiative.
  • Describe key findings from the pilot and their implications for improving the utility of health information exchange data for public health surveillance and supporting initiatives to improve population health.
