Northwell’s Integration of Data Transforms Healthcare for Eight Million People
HealthShare’s interoperability platform enriches process, connectivity, and accessibility of key patient data for New York’s largest integrated health system
With the evolving changes in healthcare payment reforms and the shift towards accountable care, Northwell promptly recognized the need to provide all clinicians, from nurses and physicians, to care managers, social workers, and EMS paramedics, access to real-time information across the continuum of care.
Over the past few years, Northwell has partnered with InterSystems to build a real-time repository of clinical and administrative information. In a relatively short amount of time, over 1 billion data points were generated for 11 million+ patients, from nearly 100 disparate clinical and billing systems.
This large repository has helped Northwell deliver the right information, to the right provider, at the right time through a number of solutions that are within the InterSystems HealthShare product, as well as through Northwell’s custom applications built on the InterSystems platform. Solutions such as the integrated Clinical Viewer, provide a window into the patient’s story with just one click, within one single system. Northwell’s goal is to simplify clinician workflows through fast and efficient delivery of pertinent patient information directly into the primary care providers’ electronic health record, which has been enabled through complex integration powered by the InterSystems Ensemble engine.
InterSystems’ HealthShare solution has also enabled complex care management workflows, including the ability to notify care managers about emergency department admissions of high-risk patients and changes to their risk scores. InterSystems solutions allow for advanced patient matching which facilitated Northwell’s development of its own Care Management application. The compliment of solutions has helped Northwell improve clinical and financial outcomes for patients in various population health initiatives.
